For CFO a copy of your tax return (redacted), the 1040 form.   

 

Embassy is good with transcripts but for CFO ... K.I.S.S.

No the advice/consent is not required, it is a suggestion as CFO is known to ask for it in some cases.

 

If you fiancee is shy, timid or meek ... she could have issues at CFO.   Those that are self-assured, confident and look the counselor in the eyes when speaking have fewer issues.

 

 

From the guide: 

 

** From the Family Code of the Philippines**
 (CFO does not always ask for this, but just in case)

It states;  21-25 years old for advice and 18-21 years old for consent.  Bring the ID of the parents, plus copies, the newer postal ID should be adequate .
 
An attorney in your area should have this form, being they will also have to notarize it for you.

My now wife,  then fiancee was 22 at the time of CFO and was not asked 

YMMV
