? Just write a cover letter saying the application is being made under INA320. I don’t have a template but that’s what I did, it doesn’t need to be more than one sentence.

Thanks for the info. I did the same thing. Some parts of this form is confusing. One last question before I submit my application, My son is under 10 years old and I`m filling his N600 application. I put my name in preparer section for this application. Not sure what to put in this section, my or his name?
