Hi, just wondering where you all had your chicken pox jab done, My DS-3025 form says incomplete(even tho i told them i had chicken pox as a kid), so i cant just send that along with the AOS.

You dont send the DS-3025 with your AOS... you take the DS-3025 to a local civil surgeon here in the states and he/she then completes form I-693A (Vaccination Supplement) if you need additional shot then you can have then done at that time.....

Ah yea i have read on here of ppl just sending the DS-3025 and getting away without having it transfered to a I-693A, but didnt really wanna risk that anyway, just didnt know if a normal doc could do the jab(might have saved some $$), but yea i suppose getting the civil surgeon to do it will be easier anyway. I had the shots done at a local health clinic and then took the info to the civil surgeon for him to do the I-693A it only cost $15 for all the shots needed (I had not had any done before I am over to the US) but talk to the civil surgeon about what shots you still need you may find if you tell him you have all ready had chicken pox he will sign you off as having had all the shots you need....

When I took my DS-3025 to a civil surgeon so he could do the I-693a, he did actually ask me if I'd had chicken pox as a child. (Even though I'd already been asked this at the medical in London...). So maybe the civil surgeon you see will fill in the I-693a to say that you don't need the varicella jab, even though it was marked as 'incomplete' on the DS-3025? Can't hurt to try.... if the worst comes to the worst and they say you need the jab, you can get it done there and then!

My husband's immigration doctor in Brazil told him he needed the vaccine only because he's never had chicken pox. Otherwise he wouldn't have needed it. Why do you?

I think because the doctor at the medical in London (un)helpfully ticked 'incomplete' on the DS-3025 (I just checked mine and it was marked as 'complete')... but maybe the civil surgeon will still fill in the I-693a to say you don't need it if you've had chicken pox as a child. It's no different then from at the medical, really - either way they only have your word for it, so if you get a nice civil surgeon, hopefully it can just be marked on the form that it's not needed.

My husband had the chicken pox as a child but no proof, so we went to a local health clinic and took the vaccine card to the civil surgeon. It saved us a lot of money. By the way the chicken pox vaccine is 2 shots- 30 days apart. You only need the first one for the civil surgeon to complete the form.

Yan had a blood test done by our family doctor. He said almost everyone has had chicken pox. A paper was filled out that I took to civil surgeon saying that she had chicken pox .So no need for shot Accepted by the civil surgeon

I noticed that my husband's DS-3025 was marked incomplete by the doctor in Brazil. However, when the CS transferred the vaccines to the I-1693, he marked complete. Go figure...but it worked in our favor I think.

Yea mine too - mine was marked incomplete as I had MMR and the booster for measles and rubella but not mumps - but when I went to get the I-1693 the cs told me that I dont need a booster for mumps and marked it all as complete. She also asked if I had chicken pox before and how old was I - I told her I did and it was before i was 5 years old and that was enough info on that one.
