Another update:  The passport agency called and asked for school transcripts from the time my daughter entered schools in the U.S.  I'm hoping that means that when we provide them this evidence of sole custody, they will send the passport as she is now 18 years old and graduated from high school.  I really don't want to spend $1200 for proof of her citizenship.
