Document Cover Page - no option to print?

Hi Guys,

 

I got a notice to say that I needed to send copies of all financial and civil documents to the National Visa Center in Portsmouth, NH.  I have all the documents ready to go, but one of the steps says I have to print the Document Cover Page to go with the packet.  I have followed every direction, googled all the options, done everything to try and find this page and I just can't find it!  Has anybody been through this?  Should I take a chance and send the packet without the official Cover Page?
