Timeline

I moved to the United States on k-1 visa and got married here, after the wedding I changed my name as commonly done with the marriage certificate as my legal name change document. I acquired my permanent resident card which has my current (new) name. Now I would like to travel back to Uganda to visit however, my Ugandan passport still has my old name. I am worried this will cause problems. What do you advise?

Always book  your ticket in the name on your passport. Carry your marriage certificate to connect the passport  name and GC name

Thank you. However, I'm still worried that some officials at the airports will make it difficult and argue that I have two identities... especially at the US re-entry

This is a very common situation and one they are very used to. Nothing to fear because of that. It would be solved if you renewed your passport in your married  name .. but that’s not always easy or cheap

I considered that but I would need to be physically present in Uganda for the interview to acquire a new passport.

Yes. I understand. Your situation will not be a problem for reentry into the US
