My fiance did not receive the DS-3025 when he had his medical in Poland. They said it was against the "rules" for them to give him a copy.

We are now gathering our packet to send out for AOS, and we don't know what to do with the I-693. He has a list of his vaccinations throught his school years, but they are in Polish, and a bit hard to read which vaccination took place when. I've called around local CS to find one to transfer this, but no luck. Everyone in Chicago wants to do the full medical, ugh! :angry:

My question is, if he doesn't have the DS-3025, can they even transfer from his Polish vaccination records to the I-693? Or since he doesnt have a copy of the DS-3025, will we have to really get the medical re-done?

I found on VJ the below doctor, but saw people write that they mailed him the DS form. Does anyone know if this is possible without?

You can go to a Civil Surgeon in your area, we did, they will do the supplement for you for a small fee.

Civil Surgeons don't understand the K-Visa and like to insist on full medical, we had this problem, and the solution is to take form I-485 instructions with you to the doctor, and point out the USCIS instructions for K-Visas. I pointed this ou to the doctor and he stopped insisting on the medical and did the supplement for $35.

They will transfer the vaccination records to the supplement.

Highlight this on page 3 in the instructions and show to doctor:

Fiancé(e)s.

If you are a K-1 fiancé(e) or K-2 dependent who had a medical examination within the past year as required for the nonimmigrant fiancé(e) visa, you only need to submit a vaccination supplement, not the entire medical report. You may include the vaccination supplement with your adjustment of status application.

Thanks YuAndDan. THe CS's I called (over 50 in the chgo area!) all insisted on the full medical. One receptionist even put a Dr on the phone so I can explain why I dont need the full thing. He said if there's a line that he has to sign, he's charging the full amount regardless.

Also, how will they transfer from vaccination records if its in a foreign language?

Need to be translated to English, in our case the medical in China recorded vaccinations in an international vaccinations book that was in English and Chinese.

There are tests to determine if some of the vaccinations have been done, or are needed.

Only ones needed are MMR, Varicella (Chicken pox), and Tetinus

Do you have an international vaccination record? Although I did send Dr Arnold my DS-3025, I also sent in my vaccination record.

AOS (My case was expedited due to husband going to Iraq):
