Rejected i864, Affidavit of Support?

I had submitted i864, Affidavit of support on Dec, 11, 2019. NVC told me to resubmit form again due there was few missing pages. My question is what date should I put? Should I put current date or old date Dec, 11, 2019 on form?

If you are sending pages that need to be signed then use the date you signed it
