I am feeling so overwhelmed and frustrated now. To cut a long story short, my mom's green card was delivered to our old address, although we moved three years ago, she went for her interview with my new address, came into the country and presented the new address to customs, I changed my address at least 2 times with USCIS, I put in our new address when I was paying for her card...... and it goes on and on. I went to the old address about a week after it was "delivered" and the current occupant said she has not seen any mail for us. The post office still had it as delivered ( August, 2019) and so it doesn't look like it's been returned to USCIS.

I have called USCIS  a few times and even spoken to an officer and still they want me to pay for a new card. *sob*. My mom needs to travel back home in a few weeks and I am feeling the pressure. 

The officer said I could fill out the I 912 form for a fee waiver but the stipulations on that form does not seem very promising.

Help, please. Has anyone else had any luck with the form or this issue in general?

You could try to send a letter to your old address, and ask the current inhabitants to either send back the LPR card to USCIS, or ask for them to forward it to your new address. It could be worth a try. 
 

either way, accidents like this happen. Luckily the card is only proof of residency, status is not effected. Your mother could schedule an appointment with USCIS using infopass to get a temporary ADIT stamp for her travel outside the country.
