Sworn Declaration to Revert to Maiden Surname - Philippine Passport

Hello! posting this question to anybody here that can help..

 

Im renewing my philippine passport on Friday, 1/31/20 and in the process of getting my paperwork for the personal appearance appointment.

 

Im looking to change my name back to my maiden name based on spousal death. Spouse was a US citizen so i only have the state death cert not a psa one which should be fine I believe.

 

However, it states on the embassy website that i also need to provide a Sworn Declaration to Revert to Maiden Surname. I have no idea where to get this form, or if i can just write a signed letter or if this needs to be notarized..

 

please help!

You can contact the PH consulate and ask for a form. The LA one claims they have a sample form on their website, but they don't. You could modify this one to meet your needs.

All the consulates are terrible about answering a phone, and very slow replying to emails.

 

I did a search also and could not find a sample for the  Sworn Declaration to Revert to Maiden Surname on any of the consulate website.   I would suggest to use what @user555  linked.

 

When I search the Philippine embassy website I don't not find the Declaration to Revert back listed.   They only ask for original and photocopies of Death Certificate.

 

Additional Requirements for Change of Family Name

  • Original and photocopy of annotated Philippine Statistics Authority (PSA) Birth Certificate
  • Original and photocopy of PSA Marriage Certificate/Report of Marriage
  • Original and photocopy of PSA Marriage Certificate annotated to indicate court decision on annulment/ recognition of divorce
  • Original and photocopy of Death Certificate of Deceased Spouse (on PSA Security paper if deceased is Filipino)
