The DS260 is submitted electronically. Once it's submitted you'll be able to print out a confirmation page that you bring to the interview.

 

Otherwise, that list is pretty much it. I actually created a thread a couple of weeks ago where I double checked the list of documents that would be required (we are using a joint sponsor, so all the items in my list may not apply to you):

Managed to bag an appointment on 9th Sept - was online last night at like 12:45am so had a check and it popped up! It must have been the one you cancelled @DCB 😂

I guess access to the DS260 is only possible when in receipt of LND Number?

Correct.

 

You can, however, find an exemplar form here: https://travel.state.gov/content/dam/visas/DS-260-Exemplar.pdf. I used this to prepare my answers to the questions while awaiting my LND number.

If your summary care record has any illnesses listed, even if it’s from 10 or so years ago, I’d get a GP’s note to confirm the status of the illness. Mine had a couple from 9 years ago and I required a GP’s note. That set in a bit of panic because I had 6 working days in between medical and interview. Luckily I got them privately instead. GP still hasn’t responded despite numerous prodding.

Summary Care Record? There is no mention of that in the screen shot. I guess that doc applies to the Medical as detailed in this screenshot.

 

How old can the Summary Care Record be since the date it is provided by GP, 6 months, 12 months? Wondering whether this should be requested just now, before LND arrives?

 Per Visamedicals, they want it to be no more than 2 months old.

Given the time its taking to get a LND Number I wonder when I should request the Summary Care Record..........

Now. At least ask your surgery how you will get it and how long it will take. Some people can access online and print from home. At least find out. If you have any illnesses or surgeries that will need a write up, then get that initiated because it requires the doctor, whereas immunization records or summaries will just require staff to provide.

Hubby sent an email as well, and they gave the same, "please be patient" reply. I'm going to let the post come today and then may reach out to our congressman back stateside.
