P1, #10: the “to” date on my current physical address - do I enter “present” because it’s where I’ll be living when I file (in the next week), or do I enter the date I’m going to leave in 2/3 months?  

 

P1, #27: My father is deceased. Do I list his last city of residence or enter “deceased” in the address box? 

 

P1, #32: Do I use mother’s maiden last name, her last name as it appears on my birth cert, or her current last name? 

 

P1, #41: If I was born in the U.S (and indicated it in the 40a. checkbox), do I need to check yes, no, or leave this one blank?

 

P2, #52: Nature and Degree of Relationship:  If I enter “Fiance, Unrelated”, is this acceptable?

 

P4, #6: Hair color - my hair is brown but i have none on top, only the sides. The “bald” checkbox says “no hair”. Do I check brown or bald?
