My husband and I are filing our adjustment of status and I see it’s mention the i693 form. I know it states on the uscis website that if the medical exam was done within a year of filing then the form doesn’t need to be sent with the adjustment of status. However do I need to send the DS3025 form? My husband was never given this after his medical exam, he was only given a CD with his X-rays on it.

It was included in the "packet "  Many send it if they have it, but not required 

YMMV
