I have a question on the Passport application (DS-11) and was hoping someone could help. I have searched the internet to no avail.

Page 1 has a Name section. I am assuming this is for the person the passport is for (my son).

Page 2 has first line "Name of Applicant" Followed by the parents. Would the applicant be me (the one applying for my son) or my son?

If you don't have travel plans, then they can be left blank.

 

Also, there is an online Form Filler that allows you to enter the info and print a PDF form with a unique barcode: https://pptform.state.gov/
