Medical Record/PDF Questions

Two questions:

1. Does anyone have the editable U.S. Medical PDF questionaire? Writing it looks so sloppy.

2. My fiance went to the hospital for random things like a scar from falling off his bed, etc. so when it says hospitalized and attach information, we just make a word doc list and attach it?

Can anyone answer this? Also, we read somewhere at the medical Appointment they give him a DS-3025 to take with him to embassy, anyone know what this form is?

You read wrong. You take nothing medical to the embassy.

1. There isn’t one

2. Hospitalized in the US means checking in and staying overnight because you are really ill or had surgery. It doesn’t mean showing up at a hospital to get stitches and going back home when they,ve patched you up. If he stayed a day or to  then provide a list anyway you see fit.
