Adjustment of status/medical

hello all

i am filing for Adjustment of status petiton, she is here on K-1 visa. what needs to be done for the medical? seems i seen somthing about she only needs a Vaccinatin report? can she use the one she had in the philippines? or does she need to see a doctor here? i belive she needs to file out form I-693A. also i was going to do a change of address because we just moved, but as i read form I-865 "change of address" it say i have to send in that form if i have sent in a petition for Adjustment of Status. whitch im doing now.

You need to go ta a civil surgeon and have the them transfer the vaccination record to a form called I-693a. Also, this id a civil surgeon who will transfer the info. for only $10. i heard that you mail him the vacc. worksheet DS-3025 and they do there thing and make a copy for your records and mail it all back to u. best of luck..... here's the info...

I had my medical examination in the UK and now that I am filing for AOS, I can't seem to find any form or worksheet, other than the extra vacination i had. Is it possible that the report you are talking about could be in the sealed package I took to immigration when i entered! - not sure what i need to do next!
