Trying to figure out if I can make the July 30th deadline

Oh my goodness ... gonna give meself a bloody headache lol...

I've been taking a look at the paperwork to see if its possible to file the paperwork once I'm approved (much positive thoughts here) and a quicky flight and marriage in Vegas.

Did anyone else here get a DS3025 from their medical? .. all I received was a "Vaccination Documentation Medical Worksheet Two"

In another forum it states that you can send this in in lieu of seeing a civil surgeon and having info transferred to an I-693

I had the same question.. it doesn't appear that the CDN Dr's have that form! I had just the worksheet as well. It ended up I needed the records for my Civil Surgeon anyways- letters from The travel clinic etc where I had gotten the shots. That ended up being the proof and my worksheet went in the garbage basically.

there is a difference in the form here and the form that my panel physician used

instead of stating "U.S. Department of State Vaccination Documentation Worksheet"

it states over in the right hand upper corner "For Use in Canada only" and is called a Vaccination Documentation Medical Worksheet Two"

the DS-3025 says that it is for Use with DS-2053 To Be completed by Panel Physician Only

and mine says "For Use with Main Medical Form To Be Completed by Panel Physician Only"

I got the same sheet as you Lynamon and I had no problems to have it transcribed to the I-693A by a civil surgeon here in the US.

Hi Lynamon I have the exact same form you describe. There aren't any type of form numbers on it
