Filling i130 online for my spouse, and need help for some doubts?

Hi there!

I'm a green card holder, lives in Texas; is in the middle of filling i130 for my spouse. We married on the 1st week of this December in our home country. As my wife has the B2 visa, she traveled with me and currently lives with me. I have some doubts on some question in i130 form. And would like to seek help from VJ community. 

1. If any field is not applicable to me - should I type 'Not Applicable' / 'NA' in the fields or should I just leave it as blank?

2. Do I need to fill everything in CAPS? Or just the names?

3. Currently she is with me on B2 visitors visa. If in this case, should I need to mention my USA address in 'where the beneficiary lives now?' field? or the home country address? 

4. To the question, "Where did you and your spouse last live together?" should I mention my USA address?

5. To the question, "At which USCIS office will the beneficiary apply for adjustment of status to lawful permanent resident?" should I need to fill this field? Note: We will be doing visa process through consular processing.

 

It would be a great help if anyone could answer my questions.
