Does USCIS require address proofs of all the last 5 addresses of the beneficiary?

What do we do if we don't have an address proof of a place where my wife stayed for 6 months in the last 5 years ? Should we just omit that address from the forms or mention it and it's okay not to have the proof?

No proof of previous addresses is necessary or even requested.

Make sure you download the instructions for the form, and read them. They will tell you exactly what evidence you have to submit. Evidence of past addresses is not required. 

Also, never omit anything from the form. If you did that, you'd be lying, which is never a good idea in the immigration process.

Proof of address history is not required, but try to have accurate dates as much as possible. Also, your address of 6 months may actually not be 100% required to be disclosed if it was just like a vacation or temporary stay that there was no official move to a new address. Otherwise, yes, like all the other helpful members have said, you will need to disclose the history but not necessarily provide evidence of it.
