No. It is asking for the beneficiary's address for the past 5 years. So they must of lived somewhere and you state that they have for all of their life at one address. Then that will be your answer, the address to which they lived for all their life.

The screenshot that you've shown is for the questions for the beneficiary. You said "my address"... so, I'm assuming you're the beneficiary. 

If your "mailing address" from question 11a - 11i is the same as your home that you've stayed in all your life, then you can leave blank the questions 12a - 12h. However, you will need to fill out the dates (question 13). I suggest you do not put "n/a" or "none" because that implies there's no answer to this question. Notice the instructions on the question say to provide the address "if different" than the previous question (11).

I learned the hard way, I hope YOU don't have to. Advice: learn, learn, learn.
