When the sponsor fills part 3 he needs to put 1 there for sponsoring 1 immigrant .

This automatically populates Part 5 Item 1. with 1 . He has his wife and a child so he is putting 1 and 1 and item 3 and 4. The household size becomes 4 , but they're only 3 people . What do we do about this ?

Secondly my brother sponsored his wife via i-864 in 2015 who is a lawful permanent resident of the US .

Does he need to put 1 for spouse and 1 for Item 6 also ?  The guidelines say do not count any member of your household more than once .

If you only follow your own post saying “The guidelines say do not count any member of your household more than once .”, it would be solved. Don’t count more than once for the same person.

So if your brother has a wife and a child and is your joint sponsor, he must also count the immigrant. So that makes his household now 4 instead of 3.

That is correctly

He would not count his wife in both other immigrants and wife, that would be counting her twice.

If your brother is petitioning for his wife and this is not a joint sponsorship then he counts his wife under immigrant and not under wife because he already counted her.

You've entered a one for your spouse as a person not sponsored in this affidavit.  Your wife IS the person sponsored in this affidavit.  Do not count her twice.
