Fist time filing I-130

On Form I-130, Part 5, Other Information Have you EVER previously filed a petition for this beneficiary or any other alien?

I am not sure how to answer  this questions because  previously I have filed DS -160 and  applied for my mom  a visit visa (b2)  which was denied  so does DS -160 (visit visa form )count as  as petition what do I answer yes or no? this is my   Fist time filing I-130

A DS-160 is a visa application. The question is asking about a petition.

5/1/2017: Case received by Manila (1 week embassy transfer??? Lucky~)

7/13/2017: Interview: APPROVED!!!

7/19/2017: Visa in hand

8/15/2017: POE

That’s not a petition. The I-130 or I-129f is a petition.

Thanks so much can i ask one more question ?

Provide your physical address  for the last five years ,whether inside or outside other united states . and it goes from 12.a..up to 15.b and i can only put  two physical address   so  the third address i have has to go page 9  for additional information page and its asking me to put  page number ,part number, item no 

 what exactly do I put for the item number?  there is not another item number available for me to put the necessary information.    ?
