Yeah you have nothing to worry about and nothing to do with regard to vaccinations. As long as the complete box is checked and USCIS doesn't lose the paperwork (which has happened) you are good. The not routinely available mark is a waiver which means the requirement is waived.

Actually neither because the ds3025 is acceptable only if included in the medical report,  however,  if you want to provide a copy of the ds3025 with the filing you can

No she really doesn't. As long as her vaccinations are checked complete at the K-1 medical and you don't wait an outrageously long time to file AOS you do not have to do anything else as far medical exams/vaccinations/health checking.

Yes that's a waiver, as in the requirement is waived. As long as the vaccinations are complete box is checked you are good. If you want to spend the money and time getting a civil surgeon to fill out a 693 for you them by all means do, but it is in no way necessary. If you are already married and have the certificate I would be getting my AOS sent in as fast as I could.

Good luck with getting the civil surgeon to give you a brake and just sign off.
