From the USTravelDocs

Fill in all the required fields, then click “Continue”.  Please make sure the First Name field includes both your first name and given name.  For example, if your full name is Nguyen Thi Thu Trang, please enter “Thi Thu Trang” in the First Name field and “Nguyen” in the Last Name field.  Please also make sure you enter your most updated Passport Number and please do not forget to provide your phone numbers.  These fields are very important.

I just noticed this on the US Travel Docs when going through things.

On her passport and Birth Certificate her full name would be example (Nguyen Thi Anh Thao).

On the I-129F I have put

First Name: Thao

Middle Name: Anh Thi

Last Name: Nugyen

On DS-160/USTravelDocs I have put the Surname: Nguyen and Given Name: Thao Anh Thi

From what the USTravelDocs is saying that  I should of put the Surname: Nguyen and Give Name: Thi Anh Thao.

Did I do the correct way?

From our experience filling out wife’s DS-160 for a B2 visa in Japan consulate. (I’ll use the example name Nguyen Thi Thu Trang)

Submitted DS-160

Surname: Nguyen

Given Name: Trang Thi Thu

When the visa was issued.

Surname: Nguyen

Given Name: Thi Thu Trang

I’m pretty sure that the US consulate will go with the name on the VN passport. Which is why I believe USTravelDocs asks you to enter it that way. So you should be okay with what you entered.

Additionally, below is how we filled out our I-130

Family Name: Nguyen

Given Name: Trang

Middle Name: Thi Thu

The example name IS my wife's name and she completed the form as @islanderfever said.  Her paperwork, visa, and passport match and lists her as "Nguyen, Thi Thu Trang" and "Surname: Nguyen, Given name: Thi Thu Trang."

I just want to add that when she gets to the US, it will be the same as well. The Green card will be "Last name: Nguyen, First name: Thi Thu Trang" and I didn't feel like spending a ton of time and effort trying to change that so I basically just got everything else in the US with that same name: Social Security card, ID, driver license, bank account, ...
