If you have a re-entry permit, RTD etc.

Don't forget to also take the form that they sent you the scheduling notice on, with the questions on the back filled in.

Thanks for replying! When should I file the questions on the notice.. before the ceremony or at the day of the ceremony?

Instructions say DO NOT FILL OUT the back of the form for the interview till you get to the ceremony

Was told at my interview to fill it in on the morning of the ceremony and sign once there. Clearly most of the 1300 or so other people at my ceremony had done the same. Nothing much is going to change between when you leave home and when you arrive at the ceremony in terms of marriage, divorce, criminal convictions, travel overseas etc...

I just checked the form. It does not say that. It says (bold added)

“Please answer the questionnaire on the reverse side of this notice on the day of your scheduled ceremony.”

and then on the back it says:

“You MUST bring this completed questionnaire with you to the Naturalization Oath Ceremony...” and then it repeats the above again about completing on the day of.

Nothing at all on there about not filling out till you get to the ceremony, only that it must be filled out on that day.

Our letter that came with the paper said to bring it to oath ceremony and fill it in there as the immigration officer would sign it that he saw it filled out

Guessing you didn’t have 1300 people at your ceremony lol. The officers at mine certainly had no interest in waiting for 1300 people to fill out forms in front of them, the one or two I saw that hadn’t filled them in were told to go do it and then rejoin the line... no one watched them for sure. Anyway the clear point is that OP should follow the instructions if they get a letter that differs from the standard N445 form, as you seem to have.
