I filed without one. I received a USCIS account number in the Receipt Notice.

2. Did you upload G-1145 in "evidence" section?

None of those. When you pay online, the uscis website directs you the payment site and send you a receipt right away. Under your profile with your case, you can edit how you want to get notified.

I have a question in section to Name 5years travel without USA I dont have a space enough on those 5 lines, can I use more paper or Its only if you were stayed out from USA more then 6 months?

I think you can use more paper (as per instruction in filling it up the paper form). Also, you should put any place you travelled more than 24 hours outside the USA.

Yes as per the instructions you can add another piece of paper and you should add any trip where you were outside the US for more than 24 hours.

Yes you will. It will redirect you the payment site. After that, you will receive an email containing your receipt number (case number).

I Suggest you add a line at the bottom stating continued on addition page - or something to that effect. On top of the page you continue add continued from page X section Y - Travel outside the US. If I were you I would also attach the i94 records from the website (and make sure it matches the dates you write down)

Since you are paper filing I would also add an excel spreadsheet print out (showing date left and returned and number of days out and where etc -  title the sheet number of days out - include the with the total number of days calculated for them) - make it easier for them to review it

 

but that’s me - I probably have too much experience with government regulators lol
