Did you get married in Pakistan?  And if so, does the nikka namah (NADRA?) document list her maiden name?  In most US states (maybe all?), the marriage certificate serves as the official name change document and is used in lieu of going to court for a name change.  The presence of the husband's name and the wife's maiden name is enough to serve as legal proof of name change by marriage, and the wife can simply begin using the new name if she chooses to.

 

In terms of the petition, when if comes time for the interview, the consulate will have a copy of the petition as well as the marriage document, so it will not be an issue to have two different names.

Not only is the spouse name change a non-issue for the petition process, there is not even a need to notify USCIS.  Your wife can just use the married name on all subsequent paperwork at the NVC stage.  

 

Even government employees can understand that Mary Jones is now Mary Smith, if the beneficiary spouse is John Smith.

No it will not. Just do it. No need to dwell on it. Both my husband and I changed our names my during our process  No problems at all.

Yes ,we married in Pakistan, she don’t have maiden name (in NADRA, certificate) she just has her first name and last name in marriage certificate, so still is it possible to change name ??? Thank you once again
