Not common by any means, but it has happened:. See one case below. That said, I was more of thinking along the lines of misreps not directly tied to intent (like saying your purpose was to attend a conference that didn't exist).

Don't conflate "preconceived intent" with "fraud or willful misrepresentation." The following case was approved, but it seems the IO was fishing for the applicant to admit to a previous material misrepresentation during the AOS interview:

"We got a very strict immigration officer for our interview who asked and asked and asked about this particular issue of intent and seemed very unsatisfied with the answers we gave."
