how to fill out #10 of I-134

In #10 of the I-134 "I have submitted visa petition(s) to the Bureau of Citizenship and Immigration Services (CIS)" should my fiance list my name and my 2 children's name or just my name? If their names have to be listed, the space wont be enough since there are only 2 lines. How are we suppose to fill out this item, can we put their names together on the 2nd line?

Nov. 19, 2007 Mailed AOS, EAD, and AP to Chicago Lockbox

Dec. 18, 2007 AOS transferred to CSC

Dec. 28, 2007 Biometrics appointment (done)

Jan. 16, 2008 EAD card production ordered

Feb. 8, 2008 AOS touched (K1 & K2)

Feb. 10, 2008 AOS touched (K-1 & K-2)

Nov. 25, 2009 Mailed documents to Vermont

Nov. 27, 2009 ROC documents recieved in VSC

By the way, i have a copy of my fiance 2004 and 2005 tax return but it was not signed by him. Is it okay to present at the embassy without his signature?
