The VJ I-485 checklist shows the following item.

13. I-693, Medical Examination of Aliens Seeking Adjustment of Status. (If you already have this from your overseas Visa Interview submit a copy that medical check form; No fee is required in this case. If you are a K-1 fiancé(e) or K-2 dependent who had a medical examination within the past year as required for the nonimmigrant fiancé(e) visa, you only need to submit a vaccination supplement (I-693A), not the entire medical report. You may include the vaccination supplement with your adjustment of status application.)

I know we'll need updated vaccination records, and of course we have the X-Rays but I don't see any I-693 in the papers my wife and step-daughter arrived with. Is this form part of their case file?

The medical results were in the sealed envelope that you handed over to the POE. It *should* get back to USCIS, so no need for a new medical (unless they ask which sometimes they do).

ETA: They're only valid for one year though and based on your timeline, that year is almost up. You may want to consider just getting a new one done.

2007-04-16: I-485 and I-765 sent to Chicago (My AOS/EAD checklist)

2007-04-17: Received at Chicago

2007-04-23: NOA1 date (both)

2007-05-10: Biometrics appointment (both - Biometrics review)

2007-06-05: AOS interview letter date

2007-06-13: AOS interview letter received in mail

2007-07-03: EAD card production ordered

2007-07-07: EAD card received! (yay!)

2007-08-23: AOS interview (Documents / Interview review)

2007-08-23: Green card production ordered!!!

2007-08-24: Welcome notice mailed!

2007-08-27: Green card production ordered again... ?

2007-08-28: Welcome notice received!

2007-09-01: Green card received!

Done with USCIS until May 23, 2009!

Well, we're filing before the year is up, so We'll just wait and do a new one only if they ask for it.

You should have a copy of the DS-3025 vaccination worksheet. You may or may not get a RFE for the I693A which is the one that is done here in the States by a CS. They will have those forms. You might find it cheaper if you need to do any of the vaccinations, have them done elsewhere and take the record to the CS, to copy onto the I693A.
