I booked mine after receiving the letter but I am 90% sure that you can book before you get it as long as the embassy has received your case from the NVC.

If it's too early, the system will not let you in. If the CEAC status says "Ready" you should be able to get into the system whether you have a letter or not. Where is your case now? Can you fill in your timeline?

I have received my LND case number 7weeks ago and if I check my online status it's says ready but I still did not receive any letter from the embassy. I have email them couple of times not but no reply. I did fill in my Ds 160 4 weeks ago but I am stuck wandering if I should go ahead and book my medical and interview because the letter is still not here

Sounds ready to book. If the system will let you book it, you’re fine to do so. The letter may never arrive.

You do not need the letter. Read the post near this one called "Change of address after NOA2". I wrote a lot of info that covers things you may benefit from. I don't want to write it all again today.
