I have never heard of anyone getting a pass.

Maybe a child? They aren't too responsible for their own immigration. But an adult? Uphill battle for sure.

He can reapply any time (although waiting until he is 31 would be suggested to avoid the issue again).

Depending on the state he lives in he may have been automatically registered when he got his drivers license or state ID. I thought I wasn't registered until I called (847)-688-6888 and they told me I was registered. I would suggest your friend call them first just to check and make sure.

Yes, he can apply if he gets denied based on that reason. However, I read on a thread here, a man with the same situation was approved during the interview because him not registering was "not intentional". That man was from Egypt and his field office was in California. 

The immigration process caused me PTSD.

Can you find that thread? It might be useful for the OP's friend to at least read about a success story.

Obviously, specifics matter, which may or may not be the same as the OP's situation. But it's something.

Update:

 

This person had his interview this morning and was approved. The officer asked him why he didn't sign up for selective service and his answer was he was not aware that he needs to sign up. Officer told him that he believes him, and he was approved. 

The immigration process caused me PTSD.

Seriously, while this website is very good, i realized that every little thing is turned into a doomsday post apocalypse scenario by some people. Like USCIS is on a witch hunt for every little thing.
