My wife is the US citizen and we are filing the AOS application. Should she use her maiden name on the forms also do any of the documents for the AOS nedd to be signed by a notary public?

Nothing needs notarising when submitting AOS. As for the married name, I am the USC and I submitted my paperwork (my part - the I-864) with my married name. By submitting the certified copy of our marriage certificate it explained the name change very well. This is merely our experience.

She should use the name that she is going by. By what name does she introduce herself these days?

The form instructions tell you what, if anything, needs to be notarized.

I used my marry named. Not all my forms have been changed but it worked.

Met Jan 1998, vows on 2006, Jay Jay born 2008, baby 2 - 2011
