If Mary is the first name and their is no middle name and Sachez Pacquao is the last name? Then no I have never see it that way. I've see where a Filipino takes the mothers last name as their middle name.

When completing the DS 160, my fiancee's middle name on her passport is her mother's maiden name like all Filipinos. The form does not ask for a middle name. Should I use that as part of her Surname?

Not true my dude, not all Filipino's middle name is their mother's maiden name!

Not to be mean but that's kind of ignorant.

You follow the instructions that are in the letter. It says to pay the fee. If you show up at the embassy without paying the fee I can only guess that you will either asked for payment then or you will be turned away till such time as you complete the payment per the instructions that I have placed belowed that I copied out of your P3 letter.

I didn't see them myself, but there seemed to be. Earliest he could book was for jan 5, 7 , and 9, then 13th, 14th, and 15th. We opted for the 15th to give enough time for the medical results to be sent in and for me to mail him the I-134.

From which years do we need tax returns?

2018 and most Embassies are not excepting tax returns but require tax transcripts from IRS.gov.
