While you're right saying nobody is entitled to a visitor visa, I think it's sad if people cannot visit their family due to a presumption of guilt.

It is not due to a presumption of guilt, it is due to a presumption of immigrant intent, which the CO is required by law to consider.

Yes, it is sad, and it is directly due to actions by visitors, overwhelmingly from certain countries, in the past.

It will help you to know that visas are usually denied to people who do not work in the city of origin, because what they do not want is that people stay to live in the United States. One of the things that can help you is that she is in an agreement with a business or insurance association, because this way you will be much easier to authorize the visa.

The fact that she has been denied once does not mean that the second does not work, you can take her out as a resident if you need a special doctor or if any of the family requires her to be there as long as he has his documents in order.

I opened several branches of my (link removed) business in the country and that way I was accepted immediately.

Your post makes no sense, clearly some kind of underhand way to try advertise your business that was linked(replaced above in extract). Reported.
