My husband has been in El Salvador for more than 15 months he had his interview in January 2019 but originally it was for July 2018, he was told he qualifies for a visa.  He was told he had to do another 6 months with the Psychiatrist in which he completed the 6 months for a second time around.  The psychiatrist sent the results to the embassy last week in which the embassy emailed me saying they need the medical exams. This has been an on going issue and money after money he has gone to take his medical exams three times and paid three times.  He went to the psychiatrict for a year in which the first 6 months it was one visit a month and the other 6 months twice a month paying for every session we have spent a ton of money.  How much longer does he have to wait to get his visa another 6 months?

Unfortunately, without medical/psychiatric clearance, he will not get his visa.  All you can do is comply with their requests and hope for the best.  Immigration is expensive and involves long wait times.

I have no idea.  Entirely depends upon the results of the tests and evaluation by the doctor.
