Hire a nurse from the Philippines

I did this for my grandmother a few years back.  She had a major stroke and needed constant care.  Instead of putting her in a nursing facility, we renovated her house to accommodate a hospital bed, added a special wheelchair-accessible shower, and found a local RN with years of experience who took the very generous financial package including free rent, and she moved into the basement suite we built for her and her husband and new baby.  They lived in the home with her and gave her many years of a very high quality of life, not just for her but for family who came to visit frequently.  Based on the $18K per month the older couple are paying for nursing home care, they clearly have the money for an arrangement like this.  Someone just needs to help them get the word out that they're looking and I would bet that they will have many local applicants.  The older NH couple would probably spend less money than they are now and have a much better life, living in their own home.  The RN and her family that took care of my grandmother became like family to us.  Good luck!

I don’t think there is a shortage of live in caretakers in the US. They can find one in the US- no need to bring someone over.

It is a creative way for your sister to try and bypass the long lines for F-4. Why are they insistent on a Filipina? Is there a language issue that would prevent them from hiring someone of any other nationality?

Care.com claim it takes 3 minutes to find a caregiver in your area on their site. I’m sure there are other agencies locally that can help.

Timeline in brief:

Married: September 27, 2014

I-130 filed: February 5, 2016

NOA1: February 8, 2016 Nebraska

NOA2: July 21, 2016

Interview: December 6, 2016 London

POE: December 19, 2016 Las Vegas

N-400 filed: September 30, 2019

Interview: March 22, 2021 Seattle

Oath: March 22, 2021 COVID-style same-day oath

Now a US citizen!

I do not agree it is impossible, I would query the logic for a couple in their 90's to go through such a long and complicated process, time is not on their side.

And then it may be for nought, an EB3 comes with a GC so no need for her to stay around.

“If you know the enemy and know yourself, you need not fear the result of a hundred battles. If you know yourself but not the enemy, for every victory gained you will also suffer a defeat. If you know neither the enemy nor yourself, you will succumb in every battle.”

Exactly, which is why it makes more sense for her to pursue the EB3 and the couple to pursue live-in care as separate endeavors.

I think having a "nurse" (RN or LPN) provide live-in care was probably way more common when your 90 year old neighbor was practicing nursing so this is what she's going off of. Private duty nursing is what this couple is looking for and they probably could find a licensed nurse or two for the right price. Much more common these days are live-in aides. Some states do nominally certify these people to ensure basic competence but it can be kind of a crapshoot. Hopefully they can figure something out!

it covers things like au pairs and outside domestic help

Registered nurses would not fall into that category.

Filipina nurses are known to be hard workers, I have heard this a number of times from folks who live all over the country who are in the healthcare field. plus they have already had experience with the older sister (my wife). I don't think they liked the care they received from a number of the local LNA's in the past when the elderly lady had a bad fall & required in home assistance for around 6 months.

Pretty sure there are plenty local nurses who are originally from the Philippines. Or do they have to be newly immigrated to count as hard workers?

Maybe newly immigrated to count as someone willing to work for below-industry wages for the profession.
