Proof of Name change. K1

Timeline

Hi. I am the petitioner and i have been married before. it says i have to have proof of name change. I am now in a different state than the one i was married in.. they said it will take a month to get my marriage certificate by mail. I have my divorce decree. is that enough?

Does the divorce decree specifically include the verbiage about changing your married name back to your maiden name?

No, you need the marriage certificate.  You need to send proof of all marriages and how they were terminated.

You only need to provide proof of termination of marriages.  The former marriage certificate is required only for proof of name change not the marriage itself

yes, i need to a proof of name change since i kept my ex's last name.
