Question number 29a. Should I mention the name of that child who is not living with me since my divorce (since last 5 years). This is form i-130. I am beneficiary.

Already answered above. You need to list all your children regardless whether they live with you.

Mother is having Legal custody of child

they say

Should my wife mention in additional information that the child is from ex wife of my husband and ex-wife has custody of child since divorce?

Why would it? You’re listing the previous spouse, and anyway they don’t care about when kids were born and whether or not you were married at the time, or to who. It’s NVC, not a church. All they want is for you to list the details of required family members. Your child is still your child even if he or she doesn’t live with you.

Does my wife need to attach with petition child's birthday certificate in my documents?

Does my wife needs to attach the document by court proving that child's mother (my ex-wife) is having custody of child??
