EAD Approved but card sent to old address

I moved in May 2019 and filed change of address (COA) right away with USCIS and received digital confirmation for Form AR-11.

 

Today I received I-797 Notice of Action (EAD approved) to my old address, but was "yellow sticker" forwarded by USPS to my NEW address.

Upon checking case status online, it gives me a USPS tracking number for the EAD card itself showing delivered, but also to my OLD address zip code..

 

To me this indicates that my "confirmed" change of address didn't stick in their system and likely my wife's social security card will go there too.. should I file another COA with USCIS?

It seems like to avoid requesting a new EAD card the only option I have is to physically mail my old address and hope the new tenants are cool and I can pick up my mail?

As per this USCIS link, make sure to file a proper change of address with USCIS and USPS (if you already didn't): https://www.uscis.gov/addresschange
