6weeks and 3 days later i was dq'd and after 8days i get my interview latter which was after 20 days .Interview cleared and get my visa issued same day

All you need is first get a computerised nadra marriage certificate that can be obtain showing nikahnama and english translation from urdu nikahnama .you can get it from any kachari of ur area 

These three docs are mandatory, without submitting these nvc wont accept

Note: Kachehri translator people are not certified/authentic according to NVC as they need certification along the translation. 

Yes just go to Lahore courthouse (کچہری )and show them your urdu nikahnama and ask them to translate everything on your urdu nikahnama. After it’s done, read whole translation by yourself to make sure there is no spelling mistake or any other. After that go to notary office it will be there also , you may ask someone there that you want to get your document internationally notarized for USA.Best of luck

This is newer requiremen. in past, they would take nikkah nama that was English on one side and urdu on another "marriage certificate" but now they want certified copy. Certified doesn't have to be from lawyer or court. just a statement from a person translating that the person knows both language (Urdu + English) and has certified it correctly and then sign it. You can notarize it before signing. Doesn't have to be in Pakistan, you can do it here in USA as well. But Pakistan is better as you need to show original when you go to the embassy for interview.

Same happened in my case, my marriage certificate was rejected, They want NADRA attested translation/original nikkahnama. However, I only had to give an explanation for their rule for Non-Muslims stated on their website, and they accepted the marriage certificate.

go to union consel office and get marriege certificate in english by nadra it will be accepected by nvc, in my case it was accpected

Any updates?  What did you provide in place of Nikah Nama since it is missing?
