And the medical needs to be obtained at NVC upload? Or is it something you take with you to interview ?

No.  That is during the actual visa application at the embassy after NVC is completed.

Im also seeing DS 260 immigrant visa application for completing on CEAC now, is that the visa application to which you refer ?

That is the form, but the process at the embassy is later
