129F questions

Hello everyone.

I'm trying to fill a 129F and I came across to a few issues and was wondering if anyone here could enlighten me.
First the information such as names and address are total made up and use as an example.

 

My first Issue and im thinking most important is on Section 43.

Back in 2014 I did fill a 129F for my gf at the time but the relationship came to an end before the interview, I did get the NoA1  saying that the petition was approved and forwarded to the listed consulate, but we didn't move forward from this point then I send a letter to the Embassy in Manila, the USCIS  requesting to withdraw of the application but never got an answer until a year later but unfortunately I lost this letter but what it said was basically that because we didn't reply in time the application was being cancelled.
With that out of the picture my questions are

Part 1
44- Should this be my A-# or my ex-gf A-#
45a-c Should this be my name or her name
47- Approved, denied or revoked? (they did approve at the beginning but then I'm                                                                                                                                                                                                                                                                   guessing revoked?)

Part 2, my fiance address in Philippines is too long to fit on the answer box and too many city towns as well any tricks for this?
