first of all, thanks everyone for the comments.  I work remotely from home and so I am doing this from the Arctic Ocean in northern Alaska. so I had her send me "everything" they gave her at the embassy and low & behold, she just send me blank DS-3025 form that is checked at the bottom which says image.png.fdc2c657e2d98e0a12a4f366a818a77e.png

It would have been nice to have known that before I even posted this.  Now it looks like I will need to reschedule the 485 interview because I am sure I will not get into a doctors office in time.

Thanks again...all of you are awesome!

You can still go (and I would advise you do). They will just give you an RFE pending a complete I693 (or complete vaccination section of the I693 if you can find a Civil Surgeon that will comply with just that part) which you would just be required to drop off at your LFO after the interview. Dont postpone the interview. It's an easy fix after the fact.

I didn’t not have my vaccination record either. The panel physician tol

d me that through my blood test they will check if I need to have any vaccines which are pertinent to my age.

A week later the physician called me to get a measles vaccine as based on my blood test I did not have this vaccine as a kid.

So, the physician can check on medical exam day and tell you if you need to get any shots.

Share the rest of the DS-3025 if possible. We went though the similar process recently, and yes for reason unbeknownst to me in Kyiv they like not check the "electing not to be vaccinated" box for K-1 applicants unless you specifically insist on completing the vaccination. Still, assuming she brought her vaccination records to medicom, they should transcribe them. Unfortunately recently there were Ukrainian wide shortage of measles vaccine (they do not use single shot MMR as far as I know)
You might be able to find a local civil surgeon who will only file Part 10 of I-693 for reasonable fee, if that is needed, but good luck finding one. In our experience, most Civil Surgeons are not familiar with K-1 process at all and insist on doing the whole thing and charging accordingly.
