Yeah, today I got my wife's interview letter for July 19th in Seattle. A couple questions, please. It says that we need to bring the complete I-693 medical exam, including the vaccination supplement sheet. Now, she came here on a K-1 about 3 months ago....I have the vaccination supplement sheet, but as far as the I-693 medical exam info, what would that consist of. We have her x-rays that she got in Saigon, but I think thats mainly it. Also, the Vaccinaiton suppliment form needs to be sealed? What if its opened? Thanks for any help.

I see your wife was interviewed at her local consulate embassy on Feb. 2007 and was approved, so her medical might be fairly current relative to the date you filed your AOS. Your wife's medical exam from her country is still valid according to these...

With reference to I-485 application procedures page:

"Form I-693 Med exam sheet (not required if you're applying based on continuous residence since before 1972, OR, if you have had a medical exam based on a fianceé visa."

Moreover,

at the I-485 instruction page:

" Fiance(é)s. If you are a K-1 fiance(é) or K-2 dependent who had a medical examination within the past year as required for the nonimmigrant fiance(é) visa, you ONLY need to submit a vaccination supplement, not the entire medical report. You may include the vaccination supplement with your adjustment of status application."

I read some amendment (not yet final, that is if what I have read was the latest memo) from USCIS publications as regards to the validity of medical examinations of applicants adjusting status..might post another time.

I know that on my interview letter it said to bring

~A completed medical examination (Form I-693) and vaccination supplement in a sealed envelope (unless already submitted)

So as long as the medical was done within a year of filing to AOS and a proper vaccination supplement is available then you don't need to bring it. So if you did not turn in the Supplement form to I-693 Appendix A then you need that filled out by a CS and taken to your interview. If you submitted that form then you're good to go.

Yup, StormnRolly's right. And here's another relevant article from USCIS publications:

"§ 1245.5 Medical examination.

Pursuant to section 232(B) of the Act, an applicant for adjustment of status shall be required to have a medical examination by a designated civil surgeon, whose report setting forth the findings of the mental and physical condition of the applicant, including compliance with section 212(a)(1)(A)(ii) of the Act, shall be incorporated into the record. A medical examination shall not be required of an applicant for adjustment of status who entered the United States as a nonimmigrant spouse, fianceé, or fianceeé of a United States citizen or the child of such an alien as defined in section 101(a)(15)(K) of the Act and §214.2(k) of 8 CFR chapter I if the applicant was medically examined prior to, and as a condition of, the issuance of the nonimmigrant visa; provided that the medical examination must have occurred not more than 1 year prior the date of application for adjustment of status. Any applicant certified under paragraphs (1)(A)(ii) or (1)(A)(iii) of section 212(a) of the Act may appeal to a Board of Medical Officers of the U.S. Public Health Service as provided in section 234 of the Act and part 1235 of this chapter."

Okay. Thanks. But what about having to have the vaccination supplement form in a sealed envelope? I already turned that form in with my aos package, but it wasn't sealed. I also have a copy of it, also not sealed. Also, do you think she should finish her chicken pox series? She had gotten the first shot back in March, before the CS filled out the I-693, though we never followed up to get the second one completed. Thank that may be something they will ask for?

The doctor, who made the vaccination supplement form for you was not carefully. That form must be sealed and he can give to you the copies without sealed.

I think that you don't need to make any second one for her. I checked with the doctor (according to your link sent to me), he said all the vaccinations in Vietnam before we came here are enough, the wife or fiance doesnot need to take any more vaccination.

My AOS filing date was after yours 1 month, I'm waiting for the news from yours and mine.
