Civil Surgeon for vaccination transcription in Nashville, Tennessee or nearby states

Hi everyone,

 

I will soon be filling for AOS from a K1 visa, and I was wondering if anyone could recommend a reasonably priced Civil Surgeon to complete a vaccination transcription near Nashville, Tennessee. I have looked through the posts that people have kindly put together on this, but they all they a little far away. If anyone knows of somewhere close by that they can recommend, I would be very grateful.

Did you not get complete vaccinations prior to the Visa?  What is marked in the bottom left corner of the ds3025?

Hey, thanks for your reply.

 

Unfortunately not, I was told at my medical I could get them done afterwards and send the evidence of vaccine with my AOS application, but it turns out this has to be transcribed by a Civil Surgeon, and I have read stories that this can cost quite a substantial sum!
