So I went for my medical in London on the 13th of August. Now the medical in London does not use the I-693 for the medical report they use department of state forms. I know at the end of my medical I received a copy of the department of state form regarding my vaccinations. What I would like to know is what department of state form do they fill out during my medical examination that gets sent to the embassy as a medical report from everything they enquired about. I've tried contacting visa medicals about this but has proved hopeless. I would like I copy of this form filled out during the examination process for my own records, can anyone offer any insight on this thank you

I had my medical in London in July - they should have given you a copy of your vaccination chart, signed off to show the vaccs/waivers you’ve had (which it sounds like you’ve had). They also give you a receipt, to show you paid for the medical. They’re the only things you get - and you don’t need anything else. 

 

If you want a copy of the other forms they fill out and send to the embassy, I’d suggest contacting Knightsbridge to ask for them - however, they’re under no obligation to give them to you, as that isn’t included in the service you’ve paid for - you’ve paid for the visa medical, which includes them doing the exam/tests etc and then sending the details to the embassy - it doesn’t include providing you with a write up.

 

I think the only people that MAY be able to provide it are Knightsbridge though, so I’d recommend discussing with them - however, if you’ve tried that and had no joy, you probably need to give up, as whilst it’d be nice if they can give you one, you’re not owed/entitled to one. Good luck.

I think it is maybe 4 pages with the vaccination sheet.

I can only give you my experience, I had a full vaccination history from the military and they accepted that, however the guidelines they used are different to the UK military and I had to have one vaccination because I couldn't prove I'd had one as a child despite having one in the military and having the record. Your mileage may vary.
