Help filling out Form 3299 - Moving unaccompanied items

I request some guidance on filling out 3299.

We are moving stuff from Canada via UPack container. 

 

Part IV(D) is really confusing.

We have pretty much nothing bought in last 1 year, except few clothes.

What do I list in IV(D) ?

  - All Personal items?

  - Any household items < 1yr?

 - Any household items > 1yr?

 - Or, just say "See attached packaging list"?

 

Some online references just say write "See attached packaging list". But packaging list is long and contains everything .......personal as well as household.

 

Sorry, I am really confused on this one.
