We're sending our packet via FedEx, for the cover letter, LOI, etc. should I put the Lewisville address or the Dallas one (since that's the main facility)? Thanks

Don't sweat an address for the cover or loi

Yeah; the sample letters online tend to follow standard business letter format and include to and from addresses, but they're not necessary, and certainly won't be cause for an RFE or denial.

Exactly, and if someone is hell bent on including an address then CSC would another or even be the better choice as that is where it is processed.   Whatever pacifies the petitionner

YMMV
