What are the requirements for affidavit of consent and support

Is there anybody here went to Washington DC Philippine Embassy to get the affidavit of parental consent? What are the documents they ask for? My son can't get a passport without the affidavit of consent and support since my mom is the guardian.I'm scared I will miss 1 document or few when I get there. I work 7 days a week a night shift.,12 hours a day and it takes me forever to get this day off and I have to drive 8 hours to get to DC too..please if anyone had experienced to get this document to let me know. thanks.FYI their website is confusing and 404 every time click their page and they never answer their phone..ugh.
